Identity and residence

Passport (valid, with visa/residence permit if applicable)
Registration certificate for your German address
Evidence of any name changes (marriage, transliteration)Spelling differences between diploma and passport are a common cause of queries.

Medical degree certificate (original + certified copy)
Transcript of subjects and hours for the whole degreeThe equivalence assessment compares content and hours – the more detailed, the better.
Licence to practise from the country of training
Apostille or legalisation, where required for your country
Sworn translations of every foreign-language document

Employment references and evidence of activity
Gap-free tabular CV
Certificate of conduct / certificate of good standingThese certificates often expire – getting them too early is also wrong.

Registered with the competent chamber of physicians
Registered with the Versorgungswerk
Exemption from statutory pension insurance applied for – within three monthsSection 6(4) SGB VI: after the deadline the exemption only takes effect from receipt of the application.
Health insurance sorted out (statutory or private)
Professional liability checked: what does the hospital policy cover, what not?
Bank account set up
